Service Care Coordinator

Provides direct service and care coordination and/or education to patients of Yakima Valley Farm Workers Clinic.
Education: Bachelor's Degree in relevant field with no experience or Associate degree and two years relevant experience and or high school Diploma or GED and 4 years of relevant experience.
Experience: Two years of relevant experience preferred.
Licenses/Certificates/Registration: Driver's License and applicable license/certification; some positions may require program specific certification. A passing score of 9 on the ALTA exam is required for bilingual differential pay.
Knowledge/Skills/Abilities Required: Bilingual in English/Spanish preferred (may be required per some position announcements), with a passing score of 9 (see above); ability to plan, coordinate and organize work projects; ability to make professional oral and written presentations; ability to deal tactfully and courteously with the public, patients, and staff; ability to utilize appropriate channels to report pertinent information in a timely manner; ability to interact effectively with patients, families, clinic team members and partners; ability to maintain confidentiality of information; ability to complete delegated activities, projects within an appropriate time span; knowledge of the social and health agencies in the area and what they offer; ability to communicate both verbally and in writing; ability to work independently with general guidance; ability to recognize a medical or emotional crisis and refer to an appropriate provider; ability to understand and apply program requirements and applicable local, state and federal guidance; hold community membership/shared experience with community being served; knowledge of Microsoft Office; and strong organizational skills.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
