Chronic Care Trainee - Asthma Home Visitor

Recruits potential clients to the asthma, diabetes, and Prescription Assistance program by explaining the benefits of the program. Provides home/office education to low-risk clients so they may modify their environment and better understand how to self-manage their chronic disease (asthma, diabetes, eczema, and allergic Rhinitis). Charts client home visit notes via Epic system (electronic charting system). Works with medical providers and agencies to obtain required application documents (scripts, signatures, etc.) for applications. Submits completed applications and follows-up with agency or patient. Provides clients and patients of YVFWC with the needed educational information related to social service agencies available in the YVFWC service area. Receives and responds to referrals from health care providers (clinic medical staff and school nurses).
Education: High School Diploma/GED, Associates Degree preferred.
Experience: 1-2 years previous work experience helpful, preferably applicable to the specified program service area. Experience with multi-cultural and multilingual communities preferred. Must have a desire to pursue a career in the health professions. Must be 18 years or older.
Licenses/Certificates/Registration: Current Washington State driver's license and proof of automobile liability insurance coverage and must pass a criminal history background investigation (no felony convictions or DWI during the past ten years)
Knowledge/Skills/Abilities Required: Bilingual English/Spanish with competency in both oral and written skills required, demonstrates ability to communicate at level 9 on the ALTA language proficiency scale. Strong writing, computer and communication skills. Ability to work both as part of a team and individually in a fast paced work environment. Able to maintain confidentiality. Respect and interest in working with in medically underserved communities. Strong commitment to service and able to work with diverse populations. Ability to lead and teach by example.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
